Are you aware of global organizations that have created and / or implemented a software solution for monitoring if / when an employee can gain access to or return to work?

The problem to solve: Daily (at the time of entering a building) self-initiated (pull not a push to employees) software tool that can capture and track answers to key exposure and health questions. Their responses will either allow them to gain entrance to enter a company building or not. Results from answers determine a Yes/Green or No/Red status that can be visibly shared with monitor to allow entrance to the building. Capability needs to be delivered via a mobile app or kiosk approach and preferably accessed via a QRC code. Are you aware of any software application or company that may have implemented something like this that we can talk with?
